MRI-Guided High-Intensity Focused Ultrasound Ablation (HIFU) of Uterine Fibroids

 

There are many ways to treat uterine fibroids. Peri-menopausal and pre-menopausal women often seek out uterus preserving techniques and treatment methods.

 

Such uterus-preserving techniques involve myomectomy, uterine artery embolization and focused ultrasound ablation.

 

Of all these, focused ultrasound ablation is the most non-invasive. MRI guidance helps in precise localization and temperature control (Figures 1, 2).

Figure 1 (A,B): This 36-years old woman with menorrhagia had a large single anterior wall fibroid (A). MRI-guided HIFU was performed at high power for 2 hours and approximately 85% necrosis was achieved as seen on this contrast-enhanced sequence performed immediately after the procedure (B). The patient’s menorrhagia had significantly diminished a week after the procedure.

Figure 2 (A,B): This 42-years old woman with pressure symptoms had a large single anterior wall fibroid (A). MRI-guided HIFU was performed at high power for 2 hours and approximately 75% necrosis was achieved as seen on a contrast-enhanced image (B), immediately after the procedure. The patient’s pain had reduced considerably a week after the procedure and the pressure symptoms are gradually reducing.

 

The results of MRI-guided HIFU depend upon the extent of ablation, but in the vast majority of women, the symptoms start regressing soon after ablation and the quality

of life scores improve to quite an extent. If care is taken during ablation, complications related to inadvertent heating of the skin and bowel can be avoided completely.

 

The procedure takes 1-3 hours depending on the size of the fibroid. Multiple fibroids can be treated at the same time. “

Sleep Studies

Today, more and more people have problems related to sleep, either with respect to lack of sleep, disturbed sleep or snoring.

Using the Embletta from medcare, a study of the sleep pattern is performed at night, in your house or hotel room. If an abnormality is found, a trial with a CPAP (continuous positive airway pressure) machine is performed the next night to see if the sleep pattern improves. For this we use the Devilbliss RPM auto adjust CPAP machine

CT Fluoroscopy (CTF)

This is an attachment to the CT scanner, which allows us to perform real-time fluoroscopy. CTF requires special changes in the software and comes with a joy-stick, which is placed on the table, a foot-switch that allows us to be in the scanner room and to control the exposure and a flat-panel monitor that remains in the room to allow us the scans.

CTF improves our ability to perform biopsies and other procedures such as radiofrequency ablations . This has been discussed in our new August 2007 newsletter, in detail.

Cardiac CT

Jankharia Imaging has acquired a new 64-slice CT, the Somatom Sensation Cardiac, from Siemens, for cardiac and other ultra-fast imaging.

This CT (also featured at www.ctcardiac.com) performs 3 rotations / sec giving us 194 slices per second, which effectively "freezes" the heart, allowing coronary artery imaging.

The cardiac CT examination takes approximately 4 minutes on table with a 10-12 seconds angiogram time. It consists of 3 parts.

1. Calcium scoring
2. Coronary artery assessment
3. Functional assessment.
